POSITION DESCRIPTION:

The mission of the STEGH Inpatient and Outpatient Mental Health Program is to care for those who access our services, enable their participation in related decisions concerning treatment, and proactively coordinate their access to other health care services as required.

Behavioural Supports Ontario (BSO) is a provincial initiative aimed at redesigning the system of care for older adults with complex and responsive behaviours associated with cognitive impairments, mental health and addictions, dementia, or other neurological conditions living in long term care homes, retirement homes, hospitals, and community settings.  An innovative and collaborative approach has been utilized to implement the BSO project across the SW LHIN.

As a part of the Mobile Outreach Team, the Behavioural Therapist will to provide clinical assessment, behavioural care planning, and interventions for older adults living with responsive behaviours, including assessment of dementia, other cognitive impairments, mental health, and addictions, physical assessment and the techniques of behavioural approach and evaluation within Elgin County and St. Thomas.   You will evaluate behavioural care plans and recommend changes to the plan that reflects the current health status and changing needs of the client.  This position will report to the Manager of the Mental Health Care Program.

 

HOURS:

Days, Monday through Friday 0800-1600 hours.  In addition, this position must be flexible to occasional changes in hours when required to meet the operational needs of the department.

 

SKILLS & QUALIFICATIONS REQUIRED:

  • Bachelor degree and/or a Masters degree in a Healthcare-related discipline.
  • Regulated Health Care Professional who holds current active membership with designated college, e.g., Registered Nurse, Regulated Practical Nurse, Social Worker, Developmental Social Service Worker
  • Current criminal check from a Canadian Police Information Centre, including vulnerable sector check completed within the last six months
  • Certificate in Gentle Persuasion Approach, PIECES (Physical, Intellectual, Emotional Capabilities, Environment and Social) and Montessori (preferred)
  • Minimum 5 years current related experience working with seniors with dementia (preferred)
  • Psycho-social and mental status assessment skills
  • Extensive knowledge of geriatric community resources and programs
  • Extensive knowledge and skills in working with various types of dementia, delirium, mental health issues, addictions, other neurological conditions, and with people who experience an acute behaviour change that can be served by a behaviour support system
  • Ability to communicate & collaborate positively and professionally with family members, community providers, and physicians
  • Demonstrated crisis assessment and intervention skills
  • Knowledge of medication side effects
  • Ability to using problem solving and critical thinking skills to provide high quality patient care
  • Ability to learn new practice and process concepts and integrate into clinical care delivery
  • Working knowledge and skills in application of legislation including but not limited to the Mental Health Act, PHIPA ( Personal Health Information Protection Act)
  • Personal transportation with a reliable vehicle and hold a valid class G driver’s license
  • Intermediate Microsoft Office computer skills
  • Demonstrated exceptional time management, organizational, interpersonal and communications skills (both written and oral)
  • Demonstrated knowledge and commitment to patient and staff safety
  • Certification in Lean (yellow belt, green belt preferred)
  • Demonstrated acceptable previous performance reflective of STEGH's Mission, Vision, Values and Code of Conduct.
  • Demonstrated ability to attend work on a regular basis in accordance with STEGH’s Attendance at Work Program.

Successful candidates must provide evidence of a fully completed COVID-19 vaccination status prior to your first day of work.
