Position Title: Family Supports Coordinator
Compensation: $67,787 - 74,068

NATURE OF POSITION:
Under the general supervision of the Manager, Children Services, the incumbent of this position provides service navigation and support to families and persons being supported to enable a more meaningful life and enhance community inclusion.
While this position is primarily M-F days, applicants must be open to a flexible schedule with the ability to work occasional evenings and/or weekends.
KEY RESPONSIBILITIES
This position has unique responsibilities and is suitable for a proactive individual who can manage multiple, individual case files, who is highly organized and possesses strong communication, technology and computer skills.
Responsibilities include but are not limited to:

- Create and maintain a positive rapport with families/guardians by working together towards a common goal, providing support and maintaining open communication.
- Maintain current knowledge of available community services and resources to enable urgent and appropriate referral and access to services including judicial, medical, mental health, protective, rehabilitation and police.
- Coordinate support for recreational programs, such as -summer camp and school break programs, in order to create opportunities for inclusion.
- Participate in developing and coordinating the implementation of physical, intellectual, spiritual, emotional, and social outcomes plans. Actively seek and value input while facilitating integrated services and finding options.
- Facilitate and foster positive interaction with families/guardians, both individually and in groups and assist in the nurturing of independence skills to self-advocate service provision.
- Maintain current records related to the program, such as case notes and consent forms.
- Provide input to, apply, administer and monitor funding, such as Special Services At Home, CSN and Flex Funding, and assist families/guardians with application and billing procedures, such as those required for Ministry funding or other programs -
- Consult and collaborate with colleagues, clinical, medical, educational and other community services in conjunction with families/guardians to plan, coordinate, and evaluate support (such as transportation, medical appointments, respite care) using various communication methods.
- Provide and/or facilitate training opportunities as identified through the Personal Plan Transitional Aged Youth, Service and/or Care Plans.
- Schedule employees to ensure adequate coverage for after school programs, camps etc.
- Participate in staff and community meetings. This may include participation in regional and provincial activities associated with Family Support programs.
- Ensure appropriate documentation and information is maintained with strict adherence to confidentiality, legislative requirements and Agency protocols.
- Participate in professional development in alignment with the role.
- Participate in special projects as assigned.
- Advocate for and with families; participate in and bring forth the perspective of families to Agency committee’s planning and Initiatives.
- Support the functions of any Family Network and family Education Initiatives.
- Meet organizational standards pertaining to quantity and quality of work performed on an ongoing basis, performing all work-related tasks in accordance with Ministry and Agency policies, procedures, legislation, regulation and directives, and identify non-compliance for resolution.
- Participate and comply with all health and safety processes and initiatives to ensure the Agency remains a safe workplace and report any workplace accidents/incidents or hazards immediately.
QUALIFICATIONS:

Education is in a relevant field of study at the minimum level of a two-year Community College Diploma.

3 to 4 years of experience in the social services sector. Experience in provision of Family Supports or service navigation, is preferred.

Ability to communicate effectively orally and in writing.

Computer skills with proficiency in MS Office, e-mail, internet. Ability to maintain a professional appearance and positive company image.

Must meet all agency training as outlined in the agency’s Policy & Procedure Manual as well as all legislated requirements.

Ability to perform in a pressure environment, with frequent interruptions and short deadlines.
- Knowledge of principles, methods, practices and legislation related social services.
- Possession of a valid Ontario class “G” driver’s license.
