Description
The Hospital to Home (H2H) program is a short-term care initiative designed to help patients transition from hospital to home, particularly when they no longer require acute care but need extra support. The goal is to support timely discharge from hospital to reduce awaiting long-term care (ALC) and reduce 30-day readmission rates.
As a member of the care team in the transition program, this role participates as part of the multidisciplinary team with the provision of patient care in hospital and community. The RPN provides patient care under the direction of the care team and/or physicians in accordance with hospital standards, policies and procedures. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for staff, physicians, volunteers, learners, patients and families
The Registered Practical Nurse supports the delivery of quality care to individuals transitioning from hospital to home. This includes creating individualized care plans and ensuring best practice approaches to assessment, treatment, navigation, and support for individuals and families. This role provides comprehensive patient and family teaching, administers medications, obtains medical histories, and provides guidance for other professionals within the program to ensure high-quality, standardized care.

Qualifications
Education:
- Current certification or the willingness to obtain Naloxone, BLS and CPI certification
Experience:
- One (1) year of experience working in hospital, community and/or other healthcare facilities, applicants with other combinations of applicable experience may be considered
- Previous experience working as a member of a multi-disciplinary team
Registration/Licensure:
- Current registration in good standing with the College of Nurses of Ontario
- Valid Ontario Driver’s license, with a clean driver’s abstract and regular access to a vehicle
Knowledge and Skills:
- Strong knowledge and understanding of concurrent disorders and associated treatment modalities
- Knowledge, skills and experience in assessment, treatment and rehabilitation, crisis management and advocacy
- Ability to work as a team and collaborate with others including members from other community organizations
- Advanced skills in reasoning, problem solving, decision-making and workload organization
- Advanced communication skills
- Ability to take initiative and work with minimal supervision
- Proven initiative, judgement, decision making and problem-solving capabilities
- Ability to work a rotating shift schedule and flexible hours
- Physical ability to perform the duties of the position
- Reliable attendance record
