Description
Reporting to the Vice President of Patient Care Services, the Director, Transitional Care & Rehabilitation Services, will provide integrated leadership across clinical services that underpin clinical decision-making, patient flow, and high-quality care delivery.
This role is accountable for the strategic, operational, financial, quality, and people leadership of both the Acute Care Medical unit, hospital to home (H2H) program and rehabilitation services.
The Director provides leadership that supports exceptional patient-centered care, efficient patient flow, integrated discharge planning, rehabilitation excellence, and effective utilization of healthcare resources across the continuum of care. The Director works collaboratively with physicians, managers, clinical leaders, community partners, Ontario Health, Home and Community Care Support Services, Indigenous health partners, long-term care homes, and other regional healthcare organizations to improve transitions of care and support patients in achieving optimal functional outcomes and successful discharge to the most appropriate care setting, optimally supporting patient to remain in or return to their homes.

Qualifications
Education
- Master's degree in Health Administration, Nursing, Rehabilitation Sciences, Business Administration, or a related healthcare field preferred.
- Regulated health professional designation required (e.g., Registered Nurse, Physiotherapist, Occupational Therapist, or equivalent regulated healthcare profession).
- Current registration in good standing with applicable professional regulatory body.
Experience
- Minimum five (5) years of progressive community based allied health, long term care or healthcare leadership experience.
- Demonstrated experience leading rehabilitation, continuing care, transitional care, patient flow, or related clinical programs.
- Experience in hospital operations, quality improvement, budget management, and human resources management.
- Experience working in a unionized healthcare environment.
- Demonstrated knowledge of Ontario healthcare system priorities, including ALC reduction strategies and integrated care models.
Knowledge and Skills
- Strong understanding of rehabilitation services and acute care programs.
- Knowledge of Ontario legislation, standards, and healthcare funding models.
- Demonstrated ability to lead organizational change and quality improvement initiatives.
- Excellent interpersonal, communication, facilitation, and conflict resolution skills.
- Strong analytical and decision-making abilities.
- Proven ability to develop partnerships and collaborate across sectors.
- Proficiency with healthcare information systems and performance measurement tools.
