ABOUT THE ROLE

The Pain Management Program requires a highly organized team player who will join our interprofessional team to assist us in carrying out the mission of St. Joseph's Care Group. This role requires a demonstrated appreciation of people-centred, strength-based care, self management principles and the complexity of chronic pain. It also requires an appreciation of accessibility accountability best practices and evaluation.

WHAT YOU BRING

Education/Experience:

• A degree or equivalent from an accredited Physiotherapy program
• Current unrestricted registration with the College of Physiotherapists of Ontario (CPO)
• Knowledge and experience in working with components of chronic disease self-management chronic pain and mental health concerns required

Skills/Abilities:

• Excellent assessment and intervention skills are required
• Ability to manage a clinical practice in a health care setting
• Ability to complete analysis, using clinical knowledge, skill and judgement to synthesize assessment information
• Ability to communicate effectively and concisely, both orally and in writing
• Ability to work independently, interdependently, and collaboratively within a model of interprofessional care
• Ability to develop and maintain respectful partnerships
• Demonstrated commitment to People-Centred Care approach and principles
• Strong project/time management, analytical/critical thinking, problem-solving, and decision-making skills
• Ability to use independent judgment and maintain confidentiality of records, actions, and information
• Excellent assessment and intervention skills are required
• Demonstrated regular attendance at work

Asset:

• Demonstrated community-based experience and working with primary care in early intervention in chronic diseases would be an asset

Conditions of Employment:

• This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
• Will be required to travel to various work locations and provide own transportation
