The Nurse Practitioner works both independently and collaboratively as part of the interprofessional team to provide advanced nursing and primary healthcare services to adults with complex and chronic health needs, including individuals living with dementia, mental health conditions, acquired brain injuries, substance use health challenges and cognitive impairment or decline. The Nurse Practitioner will deliver quality care utilizing the full advanced scope of practice, and promoting, engaging-in and leading: educational opportunities, research, quality improvement, leadership initiatives and consultation/ collaboration as outlined by relevant policies and procedures and the College of Nurses of Ontario’s Standards and Guidelines.
The Community Adult Mental Health Programs aim to prioritize dignity, autonomy, and safe transitions. The stepped-care approach matches client needs to community-based support, transitional support, acute inpatient and complex continuing care. This is a comprehensive, wrap-around service aimed at improving person-centered care, reducing caregiver distress, and enhancing continuity of care.
Skills and Qualification Required:
- Current unrestricted registration with the College of Nurses of Ontario (CNO) as a Nurse Practitioner (RN(EC))
- Completion of an approved and accredited Nurse Practitioner Program
- Two (2) years of recent related experience in community mental health; including working with clients who have severe and persistent mental illness and older adult clients with neurocognitive changes
- Experience and confidence working within a broad range of non-traditional, community settings and with a variety of service providers
- Experience supporting older adults and their caregivers
- Non-Violent Crisis Intervention and PIECES training, preferred
- Experience in student supervision, an asset
Position Responsibilities:
- Works to full scope of practice as a nurse practitioner within the auspices of the Public Hospital’s Act, and relevant policies and procedures of Providence Care
- Completes admission, discharge, prompt assessments/stabilization, diagnostic and therapeutic interventions, and provides timely feedback to the interprofessional team as well as clients and family members
- Consults and collaborates with physicians, the interprofessional care team, long-term care and other members of the clients' circle of care to optimize care delivery
- Initiates and performs a wide variety of investigations, diagnostic procedures and contributes to development of collaborative care plans; interprets diagnostic and screening tests
- Completes, documents and communicates comprehensive and focused physical and mental health assessments on clients
- Communicates assessment findings and appropriate medical diagnosis to clients and their families
- Prescribes and initiates medications and therapeutic interventions as necessary
- Acts as a clinical resource for members of the interprofessional team, keeping abreast of current legislation, regulations, leading practices, practice standards and guidelines
- Facilitates client/family access to resources to meet their goals and provides health and wellness teaching and chronic disease management as necessary
- Participates in providing education, mentoring and supervision to student Nurse Practitioners
- Fosters lifelong learning in the work environment
- Routinely precepts Nurse Practitioner students and other nursing and non-nursing students as relevant to their role
- Participates in discipline-specific and organization committees as requested
- Participates in continuous quality improvement and accreditation activities
- Leads, collaborates or participates in nursing or other approved research projects
